Repairing your teeth
Will Invisalign need refinement scans? What to ask before starting
Invisalign planning may include additional scans or aligners. Find out how refinements fit into a proposed treatment plan.
Talk through your options for repairing teeth
If you would like to understand how a tooth might be repaired or protected, a £30 consultation offers time to discuss your concerns and what an individual assessment may involve.

What does Diamond Apex Invisalign Provider mean?
Diamond Apex is an Invisalign provider tier recognising high annual treatment volume. It reflects extensive experience with the Invisalign system. Your treatment is planned around your teeth, bite and goals, with clinical reviews and retention discussed from the outset.
You may need further scans and aligners during Invisalign treatment if some teeth have more movement left to complete. These stages are often called refinements. Before starting, it helps to understand how progress will be checked and what further treatment could mean for your appointments, fees and overall plan.
A new scan documents a new starting point
A new scan records where your teeth are now. If their positions differ from the original plan, the clinician can use that record to reassess what movement is still needed and why. The scan supports the decision; it does not, on its own, mean that you need more treatment. [1]
Distinguish the cause from the new appliance
The reason for a refinement matters. A tooth may need a difficult movement, the aligners may not have been worn as planned, or the agreed endpoint may need discussion. Treatment planning, case complexity and patient-related factors all influence the result. A further set of aligners should come with an explanation of what it is intended to address. [2]
Review the revised goal
Before agreeing another stage, you should understand the planned change, its expected benefit and whether any additional techniques are needed. The British Orthodontic Society notes that complex movements may need more than a routine aligner approach. A revised digital image is still a treatment plan, with limits, rather than a guaranteed result. [3]
Agree the practical consequences
A refinement can affect visits, active treatment stages, fees and the plan for retainers afterwards. These practical details should be clear before you agree to proceed. If the plan changes, the clinician needs to explain the change and obtain appropriate consent, including a discussion of alternatives and whether the likely benefit is worth the additional treatment.
Rotated premolars: why aligner movements may need refinement
Rotation needs its own progress check
Straightening a rotated premolar is not the same movement as tipping a tooth in one direction. Aligner evidence shows that predictability varies between movements, with rotation among the less predictable movements in the studies reviewed. A clinician should assess the actual position achieved rather than treating the digital image as proof that rotation has been completed. [2]
Review the whole plan before requesting more trays
If a premolar needs further rotation, the clinician will assess its position and how the aligners are applying force before recommending more trays. The systematic review of Invisalign mechanics emphasises this link between planning and the forces used. You should be able to see which part of the tooth's position still needs to change and how the refinement is intended to help. [1]
Give useful information about appliance use
Tell the team how the tray fits and whether the prescribed wear routine has been difficult to follow. Aligners move teeth through a planned sequence, so a problem should be discussed before independently changing that sequence. A refinement discussion should distinguish clinical movement needs from practical appliance-use problems. [3]
Decide what the next stage should achieve
For each further stage, agree how progress will be assessed, whether another approach is reasonable and how the decision affects treatment time and fees. Additional aligners may help complete the agreed movements, but cannot guarantee that every movement on the simulation will be reproduced exactly.
