Orthodontics when gum support is reduced or recession is present
Moving teeth through a mouth with active gum disease can worsen an already vulnerable situation.
Talk to us about straightening your teeth
You might have a particular change in mind, or simply want to know what is possible. At a £30 consultation, we can talk through your goals and explain how the next steps are planned.

Orthodontics may be possible with reduced gum support, recession or previous gum surgery, provided the current condition and proposed movements allow it. Active gum disease needs stabilising before elective movement because forces can worsen an already vulnerable situation. Recession does not always mean periodontitis; tissue thickness, bone support, inflammation and movement direction all affect the assessment.
Can you straighten teeth with gum disease or reduced bone support?
The team measures pockets, recession and mobility and checks whether previous gum treatment has controlled disease. Reduced but stable support does not always prevent careful orthodontics, while uncontrolled periodontitis increases the risk of further attachment loss. A joint plan should establish appropriate forces, cleaning access and a reliable maintenance schedule, with baseline measurements and monitoring during treatment. Gum health is therefore part of planning and consent, rather than a separate issue to revisit only if symptoms arise.
Enfield Dental Guide: Tooth crowding with recession concerns
For crowded teeth with recession, the assessment considers tissue thickness, inflammation, brushing trauma and where the roots sit within supporting bone. A photograph cannot establish bone thickness or areas where the bone covering a root may be deficient. Pushing incisors forwards to avoid extraction or enamel reduction may increase vulnerability in some mouths. Stabilisation, modified movements or periodontal input may help reduce avoidable stress. For Enfield patients, the plan should explain the intended tooth angles, the limits of movement and whether specialist advice is appropriate. Straightening cannot promise to repair lost gum tissue.
Goffs Oak Dental Guide: Assessing tissue health before orthodontics
Before appliances are fitted, plaque, bleeding, recession, mobility and pocket measurements establish a baseline. X-rays are used when indicated to assess support and root positions. Preventive care or gum treatment may be needed first, and the cleaning routine must remain workable once appliances are present. Poor cleaning around brackets increases the risk of white marks and gum problems. Goffs Oak patients should receive the same individual assessment of safe movement and follow-up as anyone else, with treatment time based on the clinical needs.
Orthodontics after previous gum surgery
Previous gum surgery does not automatically rule out orthodontics. Current stability matters more than how long ago the surgery took place, and support may remain reduced despite successful care. Old recession does not necessarily mean active disease, while a grafted area is not immune to future damage. The team reviews pockets, recession, attachment levels and maintenance history, with periodontal input where needed. Inflammation or mobility may call for changes to forces or goals. Absence of bleeding alone does not establish that unrestricted movement is safe.
The direction of root movement matters
For example, moving a root towards the edge of its supporting bone can have different implications from moving it away. Baseline photographs and gum measurements help monitor a site with thin tissue or recession. The direction and extent of movement need to be considered explicitly, with coordinated care and informed consent after stabilisation where appropriate.
Planning movement around stable gum support
Choose a plan that links gum stability, safe tooth movement and continuing reviews. Access to appointments in Enfield or Goffs Oak can help with follow-up, while the recommendation itself should follow your clinical findings. The team should explain how orthodontic and periodontal monitoring will work together.
